CRNP – Community Based Palliative Care

The CRNP for the Community based Palliative Care program works in collaboration with primary care providers, the Medical Director of Hospice and Home Care, local nursing and personal care homes, patients, families, and staff to deliver comprehensive, palliative care services to patients under the direction of the supervising Medical Director. Responsible for providing patient visits for palliative care patients, both new and established, and coordinating their plan of care. Patient visits will include visits to patient’s homes, facilities and hospitals. In addition, Community based Palliative Care Nurse Practitioner will engage in independent decision-making about a patient’s healthcare concerns and recommend palliative services or Hospice when indicated. Master’s degree from four year college or university and current PA Certified Registered Nurse Practitioner License required. CHPN or equivalent preferred. Valid driver’s license, personal vehicle for on the-job transportation and proof of insurance coverage required. Minimum of 2 years of clinical experience in Palliative Care, and/or Hospice, Pain & Symptom Management, healthcare in a hospital, clinic, home care, or physician practice setting preferred.
