The Community Health Nurse provides quality, person centered health awareness and prevention to the community. . This individual synthesizes nursing and public health practice to promote and preserve the health of individuals, families and groups. Through collaboration with clinical team members, the Community Health Nurse works to implement participant care plans and to manage and coordinate continuity of care. In carrying out this mission, the Community Health Nurse will improve community knowledge, and establish and increase participation in Chan Soon-Shiong Medical Center at Windber initiatives, and participate in activities focused on health promotion, disease prevention, care for acute and chronic illness for the community. The Community Health Nurse will act as key stakeholder in Community Health Needs Assessment Project. BSN required. Two to three years of community health experience preferred. Certification as a Community Health Nurse a plus. CPR certification required.
