This position is responsible to effectively represent and provide information about CSSMCW services to health providers, referral sources, patients and the community. The liaison will provide community knowledge and education and participate in activities focused on health promotion, disease preventions and chronic and acute illness. The liaison will be a key stakeholder in the Community Health Needs Assessment. Excellent verbal and communication skills required. Bachelors Degree in Nursing required. Valid PA drivers license required.
