This position is responsible for providing total patient care for each home health patient that is based on the individual patient care plan and medical care plan with appropriate age-specific assessments, interventions, evaluation and care implementation. This position is also responsible for providing patient and family education as the foundation of nursing care. Valid PA licensure as a RN and PA driver’s license required. Current CPR Certification required. Two years of Home Health experience required.
